Last night my 2 year old son said his penis is hurting him, when I looked at it I saw a small white patch and around the patch was red. He is uncircumcised and I am really worried. Could you please tell me what this could be and how I can help him?

This is called smegma. It is produced by the glands of the penis. In a uncircumcised male it is produced under the foreskin. When he bathes, you should gently pull back the foreskin and clean well with soap and water. It is not dangerous and is nothing to worry about. One just needs to gently retract the foreskin on a regular basis to ensure cleanliness.

I cleaned it well with a wash cloth which was painful for him and the white patch did not come off. My son keeps holding his penis saying it hurts. Around the patch is red also

It is going to be painful to clean and going to be painful to pull the foreskin back. Try to let him play in the bath tub for a little while to loosen it up. Then retract the skin and try again. When he leaves the tub, you can apply some cream if you have any in the house - like the ones you would use when changing diapers for rashes - it is soothing. The buildup has caused the foreskin to be "sticky" and it is not going to be the most pleasant experience to fix. Once it is removed, regularly retracting the foreskin during baths and cleaning well will prevent this in the future.
