You give a classic description for possible strep throat. If he is has intolerable throat, head, or skin pain; a fever of 105 degrees Fahrenheit or higher; signs of uncontrolled diabetes (e.g., marked glucose or ketone level increase, change in awareness or consciousness); or marked trouble swallowing (especially if he cannot manage enough liquids to urinate at least once every 8 hours), then an ER visit today is warranted. If he is otherwise doing well, then this could probably wait to see the primary doctor tomorrow for a throat swab culture. Continue to coax liquids, use Advil three times a day for ache/fever, and use the anti-itch cream if it helps.
