You may have had what we call bony islands in the bones or bony cysts. These can break after some time and if they go unnoticed they can form nonunions.
Also, it could be that those areas of bone just from birth had low bone density.
Others can have higher pain tolerances and injure their foot without noticing it.
The sclerotic changes probably were due to the long standing fractures.
But yes, the bone density would be the highest likelihood.
