The biggest concern is a septic prosthesis.

This is very serious if it is what is happening.

Take her to the ER NOW for evaluation.

Why have you not simply asked what her diagnosis is? After 10 days?

endocartitis

These are two main possibilities.

Less likely, kidney infection, bone infection, hidden malignancy.....

Ask them about all of these.

Can endocarditis happen after knee replacement all of a sudden?

It can, and it can present like this, and be missed.

Echo cardiogram would be needed.

They can also do repeated blood cultures, if they have not.

At least 3 sets.

They can CT her abdomen, brain, or anywhere else if indicated by exam.

A bone scan can show bone infection.
