I'm 46 and started HRT 6 months ago after menopause. A couple of weeks ago I noticed my left armpit lymph node is swollen and I have a small tender cyst under my left breast where I have scar tissue from a breast implant removal surgery that was 11 years ago. Now I noticed 7 or 8 blood looking red spots scattered across my left breast. They are flat and are not sore. My left breast feels much like it did when I had mastitis nursing my son 13 years ago. I haven't had a mammogram in two years. How fast can a hormone fed breast cancer spread and what are the signs?

you could have mastitis which would be more likely than a hormone driven breast cancer.

there are rare inflammatory breast cancers which cause redness of the breast skin and swelling as well and need a biopsy of the breast skin to diagnose. but these are rare

you should see a doctor about your breast changes and see if a course of antibiotics will help your red breast symptoms.
