Hello, Dr. David. Here is the situation. I have a dear friend in Russia who recently had a radical prostatectomy in Israel. Upon returning home, he received the pathology report in connection with tissue samples collected during the surgery. He now has to make decisions about the type of further treatment and would like more fully to understand the lab report and its implications regarding prognosis. I understand and will explain to my friend that conversation on this site does not constitute a doctor-patient relationship. He is fluent in English. Would you be able to converse with him via this site, if he applies in the usual way?

this is a very aggressive prostate cancer.first you need to see if the prostate cancer has spread to bone. a bone scan is good, but what is better is a sodium floride PET scan to look for bone mets. if you have bone mets, then you only need the lupron anti-testosterone therapy. if you don't have bone mets, then you should get pelvic radiation therapy for 5 weeks combined with the hormone shots for at least 2-3 years.
