you have to see a gyn doctor who takes medicaid insurance to have surgery to have your tubes untied.
you can contact your state's medicaid information center or social services center to see which gyn doctors in your area take medicaid insurance.
you may have to travel to a larger university medical center where they most likely will take medicaid insurance.
