Yes, you would count the first day you spot as the first day of your period.
And, since you have been trying for 3 years - you really should get evaluated for a few problems that couples have trying to get pregnant.
1. You should get your thyroid checked with a TSH thyroid blood test
2. You should have your husband get a sperm /semen analysis to check for any abnormalities
4. and, you should be checked for any possible thrombophilias (these are blood clotting disorders)
3. You should be checked for PCOS - polycystic ovarian syndrome
These are the common reasons why women can not get pregnat.

We haven't been trying since I went off the hormonal birth controll. We used condoms religiously during that time.

Well I think my first statement then helped when you should start counting
My thryoid is low and I take medication daily for it. And in the past 6 months my levels got lower, due to human error I think. I have just had another blood test to check yesterday. The last 3 months I have never missed a day.

And, then I would also make sure you are taking prenatal vitamins too
