My father-in-law is scheduled for an NP testing/evaluation. What is this?
JA: Have you seen a doctor about this yet? What medications are you taking?
Customer: Yes, his neurologist referred him to have it done. I do not have his med list in front of me, but can get it.
JA: Anything else in your medical history you think the doctor should know?
Customer: He was diagnosed with dementia 3 yrs ago, had his license taken at that time. He lost his wife of 71 years in Nov 2016 & since then, the dementia has rapidly progressed as well as some paranoia, threatening family members & increasing behavior against drs orders, particularly alcohol consumption & being "brain washed" by a predator, encouraging him to cash in ins. policies, liquidate stock acts and empty bank acts. Were advised by the police to seek a conservatorship, but having difficulty getting required documentation from his doctors
