This answer was rated:

My sister has intense stabbing pain in her toe that has a bunion. The pains come about every 3 minutes and is so bad she screams. We have tried heat, cold and putting a spacer between her big toe and second toe. Nothing works. She has also taken 800 mg of Ibuprofen.

Neuro Doctor :

Hi, this is Neuro Doctor, an American Board Certified Neurologist with vast clinical experience, and I am very glad to help you with your question.

Neuro Doctor :

Is there any blister, swelling, rash, or any signs of infection in the bunion area?

Neuro Doctor :

Has she tried any pain med other than Ibuprofen?

Customer:

No, there is nothing apparent. She hasn't tried any other pain medication. She also has neuropathy in her feet.

Neuro Doctor :

Is the neuropathy in both feet?

Customer:

Yes

Neuro Doctor :

And is this stabbling pain coming from bunion area or from both feet?

Customer:

It is coming from the bunion area only.

Neuro Doctor :

Okay I got it.

Neuro Doctor :

We need to try stronger pain meds

Neuro Doctor :

For the severe stabbing pain she can try one of the nerve-pain pills like Gabapentin or Pregabalin, or Amitriptyline

Neuro Doctor :

These can help much better than Ibuprofen for severe stabbing pain like this

Customer:

OK. Is it likely that is will stop or do you think this is going to continue?

Neuro Doctor :

She can also try Pain pills like Tylenol#3, or Vicodin etc the opioid pills which also can help for this severe stabbing pain

Neuro Doctor :

The pain may respond and subside

Customer:

Is there anything I can get over the counter?

Neuro Doctor :

but if it continues then she may need surgical options too

Neuro Doctor :

has she tried special shoes, padding etc?

Customer:

Yes, but this pain just started an hour ago. She hasn't had anything this intense before.

Neuro Doctor :

Okay I see

Neuro Doctor :

has she tried Myoflex or Bengay pain relieving skin ointment?

Customer:

No, this is new. We could try that.

Neuro Doctor :

Okay then these are over the counter pain creams

Neuro Doctor :

these are effective

Customer:

Do you think that there is a nerve that is pinched?

Neuro Doctor :

they are over the counter

Neuro Doctor :

unlikely

Neuro Doctor :

pinched nerve should not present locally in the toe like this

Neuro Doctor :

but she needs a clinical examination too

Customer:

What do you think could be causing the pain?

Neuro Doctor :

if this does not subside

Neuro Doctor :

the bunion itself can cause such pain

Neuro Doctor :

or temporary worsening of neuropathy

Neuro Doctor :

if pain continues she needs clinical examination too just to make sure nothing else was missed

Customer:

Would Oxycodone work?

Neuro Doctor :

yes

Neuro Doctor :

in fact I had mentioned similar meds earlier above

Neuro Doctor :

one contains codeine another one contains hydrocodone

Neuro Doctor :

oxycodone too can help

Neuro Doctor :

Percoset is a common brand

Customer:

Would heat or cold help too?

Neuro Doctor :

tylenol #3 contains codeine, vicodine contains hydrocodone

Neuro Doctor :

yes

Neuro Doctor :

heat or cold is helpful too

Neuro Doctor :

she should continue these general measures along with meds

Neuro Doctor :

or cream mentioned above

Customer:

OK - we will do that. Thanks

Neuro Doctor :

You are welcome

Neuro Doctor :

I hope the above options help her immensely

Neuro Doctor :

Do you have further questions?

Customer:

No, I don't.

Neuro Doctor :

I hope she recovers quickly and completely from this severe pain, all the best!

Customer:

Thanks again

Neuro Doctor :

You are welcome! All the best!
