I have pain that radiates from left hip down leg to knee, sometimes includes knee. I have been told it is bursitis. I cannot sleep on my left side without pain. when i sit i must shift so no pressure is on that side. I was told put heat on my leg what else can i do and is it bursitis?
Optional Information: Person's Gender: Female Person's Age: 54 Already Tried: heat ice sometimes heat makes pain worse.
Hi, this is Dr Sathya. Thanks for your question and I am very glad to help you.
First of all you need a firm diagnosis. While bursitis could be one of the causes for this kind of pain, however you may also have certain nerve injuries too that can explain these symptoms and such nerve injuries include;
Meralgia paresthetica (lateral femoral cutaneous nerve injury)
Pinched spinal nerve (usually back pain is present, but sometimes it can happen without back pain)
You need a thorough clinical evaluation and tests like MRI scan of the hip & lower spine area, EMG/Nerve conduction test (a nerve & muscle test) to rule out the above possibilities. This way we can confidently ascertain the cause for your symptoms (bursitis or the above mentioned nerve injuries etc). Once the exact cause for your pain is identified then it will be treated appropriately.
In the meantime you may try certain symptomatic treatments that can help with any of these above mentioned conditions. First of all if heat is worsening the pain then stop using it. Continue with ice. The following are additional symptomatic treatment options for any of the above conditions you may have;
Physical therapy
Antiinflammatory medications like Naproxen, Ibuprofen
Nerve- pain pills like gabapentin, pregabalin, amitriptyline
Muscle relaxants like baclofen or tizanidine (they are muscle relaxants but do help for this type of pain)
Lidocaine anesthetic skin patches to the painful areas
Electrotherapy including TENS
Steroid injections or a course of oral steroids
Massage therapy
Acupuncture
Ultrasound therapy
Laser therapy etc.
You may please discuss all these above points with your clinic-drs, preferably with a neurologist for the further needful (clinical evaluation/examination, investigations & symptomatic treatments as discussed above) and once a final diagnosis is made it will be treated appropriately (with definitive or specific treatments pertinent to your final diagnosis).
Please ask me if there are any further questions.
Best regards,
Dr Sathya.
