My 7 year old grandson is really having serious issues in his new 2nd grade class. He is a bright and caring boy and I don't believe he is being oppositional or defiant- however, he hasn't made any friends, isolates himself from others, can't read social cues, falls out of desk, crawls under desk, slithers on floors, down halls, on carper, can't sit still, focus is very short, disruptions in routines almost make him shutdown and withdraw further isolating himself. He is smart- he can't get work done, even though he has the skills. He makes constant grand gestures with his arms and body constantly. Fine motor skills are not very good. Seems very uncomfortable around his peers. Does not pick up social cues at all.

He needs near constant redirection, even got scared of toilet flush in school bathroom and ran out with pants down. Focuses on only one of two interests and is hard to sway away from those interests. Eye contact is tough with others at times. He seems to mention worry and a bit of anxiety. Total lack of impulse control on a regular basis. Can't be still for a minute i.e. So unless reading a book he likes.

Well, from your description about his behavioural traits and mannerisms there are 2 childhood disorders which come close to defining his behavior and these two need to be thoroughly evaluated by a child Psychiatrist or child psychologist , so as to this them out or rule in one of them as the main problem leading to this sort of behavior.
The two disorders that need to be assessed are - a ) Asperger's syndrome ( Autistic spectrum disorder ) and b) Attention Deficit Hyperactivity Disorder ( ADHD).
