Dr. Kaushik :

that is good that you have reached home.

Customer:

Still

Dr. Kaushik :

when did the fugue begin to reappear?

Customer:

I feel confused

Dr. Kaushik :

what drugs are you currently taking ?

Dr. Kaushik :

can we have a revision of that.

Dr. Kaushik :

hello.

Dr. Kaushik :

are you there.

Customer:

Because first time i come here i had fugue

Customer:

So you are sure it was because of remeron?

Dr. Kaushik :

there are high chances that it was the culprit and that is why i told you to cut down the dose to 15 mg

Dr. Kaushik :

how much remeron are you taking right now ?

Dr. Kaushik :

45 mg?

Customer:

22.5

Customer:

Mg

Customer:

I feel better on 22.5 than 15

Customer:

But i feel confused

Customer:

My brain cant understand what is happening

Customer:

5 month in fugue

Customer:

Too much

Customer:

I cant neglect canada because it is real

Customer:

But i feel not real

Customer:

I feel my brain is confused

Dr. Kaushik :

and when you feel as if your surroundings are not real that is called as Derealization

Dr. Kaushik :

Both depersonalization and derealization are type of Dissociation.

Dr. Kaushik :

Please answer my question

Dr. Kaushik :

how much dose of remeron , zyprexa and clonazepam are you taking daily ?

Customer:

Remeron 22.5mg.zoloft 200mg

Customer:

Just these two

Dr. Kaushik :

why, what about clonazepam and zyprexa

Customer:

I stop taking them

Customer:

I need too?

Customer:

What do you think

Customer:

How i get out fugue

Dr. Kaushik :

do you have clonazepam and zyprexa doses with you.

Dr. Kaushik :

or can you get them here

Customer:

Clonazepam yes

Customer:

Zypreza no

Customer:

But i get back home now

Customer:

I dont need them

Customer:

Am with my family

Dr. Kaushik :

at the moment you do need at least one of them otherwise how will you get out of fugue ?

Customer:

Zoloft

Dr. Kaushik :

what dose of clonazepam do you have

Customer:

And remeron is for ptsd

Customer:

.5

Dr. Kaushik :

No it is not enough , alone it is not good enough

Dr. Kaushik :

ok.

Customer:

Ok Clonazepam how much?

Dr. Kaushik :

good then start taking half of 0.5 mg clonazepam by splitting it into half two times a day in a gap of 8-10 hrs

Dr. Kaushik :

Do mention about this to a local doctor who knowns your history about fugue.

Dr. Kaushik :

You got to take this dose for at least 5-7 days to help your fugue subside.

Customer:

I didn't understand how i take them?

Dr. Kaushik :

you have 1 tablet of 0.5 mg clonazepam ?

Dr. Kaushik :

right ?

Customer:

Yes

Dr. Kaushik :

so you got to break it into half that will make two parts , each of which will be of 0.25 mg

Dr. Kaushik :

Then take one half part in the morning

Customer:

Ok

Dr. Kaushik :

and the other half part in the evening around dinner.

Customer:

Ok

Dr. Kaushik :

continue this for a week or so and hopefully this will get you out of the fugue and depresonalization

Customer:

I see

Dr. Kaushik :

that is the feeling of unreality

Dr. Kaushik :

meanwhile continue on zoloft and remeron as they are right now .

Dr. Kaushik :

is this understood.

Dr. Kaushik :

any confusion regarding what i have suggested ?

Dr. Kaushik :

if yes please ask.

Customer:

I feel less fugue

Customer:

Thank you i will do that

Customer:

Do you think lower remeron to 15mg will be better or no?

Dr. Kaushik :

Yes

Dr. Kaushik :

i do feel that

Dr. Kaushik :

but as of now you are on 22.5 mg

Dr. Kaushik :

as per what we discussed before

Customer:

Yes

Dr. Kaushik :

so please follow on this 22.5 mg dose for a week and then come down to 15 mg dose and maintain on it

Dr. Kaushik :

this will be better for your fugue

Customer:

Ok indeed

Customer:

Thank you

Dr. Kaushik :

Take care.

Dr. Kaushik :

God bless.

Dr. Kaushik :

Regards.
