Customer Question

I just talked to my PCP. I was switching from Paxil to Celexa because of suicidal ideation and depression. I began taking celexa 20 mg about 2 weeks ago and decreased paxil from 30mg to 20mg for about 5 days and then decreased paxil to 10mg and have stayed there since because I have been feeling back pain, and the strange head sensation when I move - especially in the afternoon. My PCP recommended discontinuing the paxil immediately and if the symptoms don't stop, discontinuing the celexa. He has referred me to a psychiatrist but my first appointment is not until June 5th. I am concerned about stopping paxil abruptly. my recent bloodwork came back normal for everything except cholesterol.

can you please tell since how many days have you been taking paxil @10 mg and celexa @20 mg ?

for about 10 days

celexa for a total of 2 weeks

paxil weaning off from 30 mg - over those same 2 weeks

Dr. Kaushik :

you see both of them are ssri anti depressants ..

Dr. Kaushik :

you see both of them are ssri anti depressants..

and what your pcp did was that he was trying cross titration

that is weaning one drug off while starting anotther

but for some reason he did the weaning a bit faster ..

Dr. Kaushik :

and so currently the symptoms that you are experiencing are nothing but wothdrawal side effects

which should not scare you since they are self limiting aka temporary

so now that you have got the withdrawal side effects , they will anyways remain for another 1 -2 weeks whether or not you stop paxil or not and anyways you are at a low dos eof paxil which is usually the last dose to be on before you completely stop it when weaning it ..

so you understand what i am trying to say here.

whether or not you stop the paxil it will not make any difference to your present withdrawal side effects

so better would be to come off paxil ..

till you have a meeting with your psychiatrist..

slowing down the paxil withdrawal will not help?

and anyways how will you slow it

you are already on 10 mg

that is the minimum effective dose

you have no point of going to half of it ..

it will not make any differenc

and these symptoms will be there for another 1 week or 2

and they will subside soon..

infact the sooner you get off this paxil..

the faster this drug will wash out of your body and

faster relief from such withdrawal side effects you will get

so you will have to endure these symptoms for few days ..

say 1 -2 weeks

come of paxil immediately while continue on celexa 20 mg

and drink 10 glasses of water and take as many liquids as much possible

from the day you start it .. you should count atleast 2-3 weeks for it to start making some effects ..

I do drink lots of water - especially since starting the celexa

but the real assessment should be made only after 6 weeks of taking it ..

and the pain from decreasing paxil - feeling sick should stop in 1 to 2 weeks?

if you have felt 25 percent if improvement by that time , then this drug is worth continuing but if not then you will have to come off it and look for another drug..preferrably anogther class.

atleast it will go down in strength by this much time

are there good long term drugs from another class?

so much so that you can feel that you are improving

for anxiety, depression , chrinic fatigue and pain

which class is cymbalta?

chronic (long standing ) fatigue and pain

it is an SNRI anti depressant

that sounds like what i need - I sleep at least 10 hours a night and then take 3-5 hour naps

unlike ssri , it works on nor-epinephrine neurotransmitters

then suggest it to your doctor

that is at dose of 37.5 mg twice a day.

I just want to enjoy life again - have been mildly depressed for probably the past 3 years and then majorly depressed for the past few months
