HiCustomer

Yes it is. It is rare, but it does happen. I'm going to give you a link to find out more about it from a reputable source with their information being from legitimate research. There is lots of hokus pokus out there about schizophrenia, but we don't want that to taint the real issue at hand, which is that it is a brain disorder that can be firmly diagnosed and treated by specialists.

How would one receive a firm diagnosis? Are there specific tests that can be done or isn't really based on symptoms?

A child psychiatrist can make the diagnoisis, which would mainly be based on his symptoms, as most all psychiatric diagnoses are. The doctor will ask him and his caregivers questions and base the next move on those answers. Functional MRIs can back this up, but really are uneccesary and more for research - even though they show the problems in the brain. He may undergo a radiologic exam of his skull and brain to rule out some type of tumor which could be causing the symptoms. This is one of the major reasons why you want to go to a psychiatrist or pediatric neurologist as they are medical doctors who specialize in these types of problems, rather than a counselor or pschologist for the diagnosis. All other medical factors need to be ruled out.
