The numbers that you report are out of normal range ...the pulse rate is high ...your blood pressure is low at same time of high pulse..this may be due to low blood volume such as may be caused by dehydration ...

Have you been in the heat ? Diarrhea ?? Anemia could cause such as these numbers too....

Do you feel faint ??

These vitals signs do represent a concern however -- Yes to that ..
