The symptoms are suggestive of primary hypothyroidism.
Differential diagnosis can be: secondary or tertiary hypothyroidism, primary adrenal gland insuffciency; Resistance to TSH or thyroid hormones.
Definitive diagnosis is likely primary hypothyroidism since TSH is high in appropriate response to low free T4.
