Prior to Dec 2011 - My blood work as of May 2010 shows everything

Resolved Question:

Prior to Dec 2011 - My blood work as of May 2010 shows everything completely normal.

Feb - Tested for STDS - entire panel negative including HIV

April 2012 - First CBC blood panel shows platelets low for the first time ever, 132,000.

June - August 2012 - experienced increasing anxiety and several bouts of brain fog while working out.

Dec 2012 - Got what appeared to be a sinus infection.

Jan 2013 - Went to a Wal-Greens Minute Clinic for supposed sinus infection and nurse prescribed me a Z-pack course of
antibiotics.

Late Jan 2013 - Came back to Minute Clinic because antibiotics didn't seem to fully resolve supposed sinus infection. Put me on 14 day cycle of amoxicillin.

Feb 2013 - Experienced a patch type rash on both left and right shoulder, some white canker type sores in mouth.

Late Feb/ March 2013 - Fatigue begins, massive lethargy. I begin to juice and notice some improvement but still very exhausted regardless of amount of sleep, dietary changes, etc.

April 2013 - Go to get tested for syphilis on recommendation from Dermatologist based on history of rash - result is negative.

May 2013 - CBC done, platelets at 133,000.

June 2013 - Exhaustion continues, rapid weight loss (15 pounds in 4 weeks).

July 15 2013 - go to ER. platelets are 120,000.

Aug 22 2013 - ER visit and subsequent CT scan show enlarged spleen (15cm)

Sept 20 2013 - Hematologist runs EBV panel, antibodies present for past infection. All other blood panels are clear.

From July 18th to now, I've been tested for autoimmune disorders, cancer, blood disorders. I've had a CT scan of my brain, heart, and abdomen. Abdomen with contrast as well and the only thing that shows up is the enlarged spleen (15.36 CM at its longest point).

Recently, on Oct 24th 2013, was admitted to hospital with fever, enlarged tonsils, sore throat with small white spots, headache, light sensitivity, and enlarged lymph nodes in neck. During hospital stay, platelets dropped to 81,000. Mono spot test was negative, CMV test negative, EBV panel showed past infection as before. I should also mention I've been experiencing brain fog through out of all of this ordeal.

Could this entire last 23 months have happened because of EBV?! I do need to mention that I drank and partied heavily through 2012 and up to May of 2013. I have not drank any alcohol since and have been living stress free as possible trying to recover.

Dr. Shahzad MD :

Hi there. Thank you for your question. I will be helping you out with your queries

Customer:

Ok

Dr. Shahzad MD :

welcome

Dr. Shahzad MD :

please wait a moment as I go through your response thoroughly.

Customer:

Ok

Dr. Shahzad MD :

please wait

Dr. Shahzad MD :

thank you

Customer:

Take your time

Dr. Shahzad MD :

ok

Dr. Shahzad MD :

so what is the symptom(s) that is bothering you the most at the present moment as you type?

Customer:

I still have the swollen lymph nodes in my neck and a light headache that comes and goes and some fatigue/brain fog that has yet to subside.

Customer:

I've been out of the hospital for a week now.

Dr. Shahzad MD :

so your fever has settled, how is your blood pressure like?

Customer:

Fever is gone, blood pressure is fine.

Customer:

I have a CBC next week to check my blood platelets.

Dr. Shahzad MD :

Can you please give me a reading of your blood pressure?

Customer:

I don't have the reports from the hospital stay, but they were all in the normal ranges according to my nurses. I asked everytime they took them if they were normal/good ranges.

Dr. Shahzad MD :

Have you had any hormonal tests like tests to check your thyroid and adrenal gland?

Customer:

I have had my thyroid checked. I'm not sure about the adrenal glands, though.

Customer:

Could this be adrenal fatigue?

Dr. Shahzad MD :

I think that you should have your adrenal gland tested with synthetic ACTH stimulation test along with serum cortisol levels morning and evening

Dr. Shahzad MD :

that will help us determine if you have adrenal problem or not

Customer:

Ok, will my hematologist be the one to schedule that or my PCP?

Dr. Shahzad MD :

Your PCP can schedule that but the best person to rule hormonal problems out would be an endocrinologist

Customer:

Ok

Customer:

Anything else you can recommend based on the information I've provided? Also, do you think that EBV could still be making my spleen enlarged and causing a majority of these symptoms?

Dr. Shahzad MD :

I do not think that EBV could be causing this particularly with the negative tests that you have had

Dr. Shahzad MD :

I think that hormonal causes should be considered and ruled out in your case

Dr. Shahzad MD :

The enlarged spleen may just be a coincidental finding

Dr. Shahzad MD :

It may not indicate anything

Customer:

1.26 was my Thyroid reading on my last blood work.

Dr. Shahzad MD :

That is normal

Customer:

If there are no hormonal issues, what next? My doctors have kind of thrown up their hands.

Dr. Shahzad MD :

I think that you should also be worked up for your adrenal gland as well

Dr. Shahzad MD :

to make sure that is normal

Dr. Shahzad MD :

If there are no hormonal issues then we have to look at things like anxiety or depression that may be causing this. Do you think that you are anxious or depressed?

Customer:

I definitely have had something wrong with me since Feb, before Feb - I've never felt ill like this...

Customer:

My hematologist suggested CFS or post-viral-syndrome from the EBV.

Customer:

I'll have my hormone levels checked, I guess. It's the only thing they haven't checked.

Dr. Shahzad MD :

yes

Dr. Shahzad MD :

any more questions?

Dr. Shahzad MD :

CFS is a possibility

Dr. Shahzad MD :

but thats kind of a waste basket type of thing

Dr. Shahzad MD :

meaning that when we cannot find anything else we label it as CFS

Dr. Shahzad MD :

so I would first like to rule out hormonal things before labelling you that

Customer:

Ok

Dr. Shahzad MD :

anything else?

Customer:

Thanks for your help.

Customer:

No, that's all. I'll find an endo today and schedule the test.

Dr. Shahzad MD :

You are most welcome
Take care
Wish you all the best.
