I have done endoscopy yesterday and i was diagnosed with mild-moderate

This answer was rated:

I have done endoscopy yesterday and i was diagnosed with mild-moderate pancreatitis, and oesophagitis. H-Pylori test tho negative.
Doctor gave me following medicines:Rabeprazole 20 * x 2 , Moza 5 x2 , Amoxidenk500 x2 , Clarytas 250 x2 , Vit B Denk x 1 , Zinc supplement x 1 for 5 days.

first day goes without problem , no more heartburn , no bloatness , sleeps well. But at second day after taking pills i got "Boring sensation" at upper abdomen (never had that before) and cannot sleep due to that. Research on internet seem that Pancreatitis due to drugs. I am very worried. Doctor told me to come back after medication.

My questions are :
1 - Should i stop taking meds ?
Or
2 - as there is no H-Pylori infection , should i remove antibiotics (amoxidenk and Clarytax) ?

Dr. Natasha :

hello

Dr. Natasha :

welcome to the chat

Customer:

Hello doctor

Dr. Natasha :

1. Yes, you do NOT need the antibiotics as your H.pylori is negative. The clarithromycin and the zinc can cause abdominal pain

Dr. Natasha :

2. You should be taking prilosec or omeprazole or pantprazole

Dr. Natasha :

3. Other medication like carafate can coat the stomach better

Customer:

so i should stop taking rabeprazole?

Dr. Natasha :

4. BLAND DIET. Rice, no spicy food, no tomatoes, caffeine, peppermint, alcohol, citric food

Dr. Natasha :

No, you can take the rabeprazole- that is like prilosec

Dr. Natasha :

But stop the antibiotics

Customer:

i c , can that be causing pancretitis problem?

Customer:

rabeprazole i meant

Dr. Natasha :

No, the rabeprazole actually helps with the esophagitis

Customer:

ok thanks , how abut Moza 5?

Dr. Natasha :

In general mild to moderate pancreatitis is treated with pain medication and hydration

Dr. Natasha :

That is mosapride, correct?

Customer:

yesh correct

Customer:

so i should drink plenty of water?

Dr. Natasha :

Well, it was removed from the US pharmaceutical list years ago

Dr. Natasha :

Because of cardiac arrhythmias- it is black box medication

Customer:

oh , i am at Myanmar (South east asia)

Customer:

i c , scary.

Dr. Natasha :

So, I cannot really recommend it for you. (yes, I know you are in Myanmar)

Dr. Natasha :

I would stick with the rabeprazole

Dr. Natasha :

drink plenty of fluids.

Customer:

ok , can i drink electrolyte ?

Customer:

cause i can't eat well

Dr. Natasha :

of course

Dr. Natasha :

hydration is key

Dr. Natasha :

let me find you a pancreatitis diet list. Hold on

Customer:

I become very weak after medication (3rd intake)

Customer:

thanks.

Dr. Natasha :

If you stick with these foods, it will be best:

Dr. Natasha :

Dr. Natasha :

Can you see the pdf?

Customer:

checking

Dr. Natasha :

ok

Customer:

currently i am only eating Rice soup.

Customer:

also when i woke up , i have smell of blood in my mouth

Dr. Natasha :

you need more nutrition than that or you will not heal well

Dr. Natasha :

can you see the recommended food

Customer:

yes i see now , connection slow here

Dr. Natasha :

good

Customer:

how to take rabeprazole ? dosage right now is 1 at morning 1 at night 20mg

Dr. Natasha :

that is the right dose

Customer:

ok i have 4 more days left , so thats all i have to take right?

Dr. Natasha :

usually they have you take it for a month

Dr. Natasha :

it takes a little longer to heal

Customer:

i c

Dr. Natasha :

So stick with the diet and take the medication for a little longer and that would be the best way to recover

Customer:

i c , for both pancreatitis and oesophagitis (grade B) , only rabeprazole is needed right?

Dr. Natasha :

carafate can also help, but from your list- only the rabeprazole is needed

Customer:

ok carafate + rabeprazole ? what will be dose of carafate ?

Dr. Natasha :

1 gm twice daily

Dr. Natasha :

for 1 month as well

Customer:

ok thanks. I got very weak due to 3000mg of antibiotics a day , today is worst.

Dr. Natasha :

that is a lot of antibiotics

Customer:

so i need nutrition and liquids

Dr. Natasha :

anyway, get some rest, eat better and stick to the diet

Customer:

another question , when i take food before sleep (before medication) i got cough ferquently , that is same problem from oesophagitis ?

Dr. Natasha :

don't eat at least 4 hours prior to going to sleep

Dr. Natasha :

yes, that is from reflux and esophagitis

Customer:

i c

Customer:

after the medication finish , should i still stick to foods in that list?

Dr. Natasha :

Yes, for now- at least the next few months. I hope that I provided you with excellent service?

Customer:

thanks a lot Dr. Natasha , u gave me best service!

Dr. Natasha :

great! keep me posted!

Customer:

you should open up a clinic in our country , many of us will appreciate.

Dr. Natasha :

:-)

Dr. Natasha :

I would love to do that- when my kids grow up

Dr. Natasha :

Hope you feel better soon :-)

Customer:

thanks.

Dr. Natasha :

Hello Natasha

i just ate some salmon with rice soup and i sense pain radiating from upper abdomen to back. Should i restrict eating food for now?

You do need to eat. Make sure you are taking meds- ask your doctor for some pain meds- like narcotics too
Customer: replied 3 years ago.

Thanks alot Natasha , since yesterday i am getting so much better , no more pain (didn't take pain meds ).And no more bloatness or boring sensation.

I am Taking rice soup with boiled chicken 4 time a day (2 bowls seperated to 4 times) .

And Liquid (Coconut Juice , Horlicks) about 2-3 litres a day.

After each big meals i take Ezymplex for 2 time a day (it have Amalase , Lipase , Protease ).

Only one problem remains , Diarrhea (Mild) about 1 -2 times aday for 2 days . Stool seem to have some fat in it. I read that i should not take Pills for Diarrhea during Acute Pancreatitis. So when should I expect that to be gone?

Also should this problem become chronic? Any signs to be noticed if it becomes chronic? Right now i gain back energy and can stay like normal person, just worried.

You can develop chronic pancreatitis if you continue to have episodes.
Most people will not have this- usually this occurs if they continue to drink alcohol...
Technically diarrhea is >500gm of stool per day. So if you are not having this, then you are having loose stool and you are on the mend...

Keep me posted.

Thanks a lot Natasha, So i should not take pills for Diarrhea?

I am not having that much stool, just watery(Thick) one to two time a day, only little pain in stomach when it goes (not like pancreatic pain , just stomach pain while diarrhea) .

I do not smoke , or drink , even once in my life. And this is first time in my life Pancreatic pain happened. Endoscopy do not show stones too so i hope I will fully recover.

Thanks a lot!

Phyo.
