If you have not noted an improvement after 4 months of rest, then you need to go back for additional tests
It is possible that you may have ruptured a tendon, such as the biceps tendon when you injured yourself
You need to see an orthopedic surgeon
You may need a MRI of the elbow and/or PT
You probably have a partial tear, but it still needs to be followed by a specialist

yes, it will reveal inflammation and/or tear or other signs of injury
Pain is greater after sleeping at night an waking up in the morning.
Unfortunately, I cannot examine you from this website- I can only go by your history. But if your elbow isn't significantly better after 4 months of rest, you need a repeat examination
I would go to a specialist at this point- there is not much your regular doctor can offer you
