I do think you should be seen by your physician today. If you can not get in to see her then, yes, a trip to the ER is indicated.
The differential diagnosis list for what could be causing such severe abdominal pain in a 45 year old diabetic female is long and branching. I would feel a lot better if you had a complete examination sooner rather then later.....

If needed Urgent care could send you to the ER. I guess it all depends on your insurance/payment issues. There is a chance if you were private pay and urgent care sent you to the ER that you could get billed twice for the same "evaluation". Welcome to the hassles of modern medical care in the USA :(
But if urgent care can handle things, it is usually less expensive. You will get quality care at either place
If no one can drive you, go to the closest place or call 911. Better that then getting in an accident on the way to getting help

It could be many things, from a gallbladder problem, twisted overy (although this is usually lower), etc.
