Sharp stabbing pain under left shoulder blade. Worse today

Sharp stabbing pain under left shoulder blade. Worse today and hurts to take a deep breath. Had my gall bladder removed 3 yrs ago.

Customer: Thus has been a dull ache for a week. After my pilates class on Thursday it was worse and today us pretty bad. Cant get comfortable or get a good deep breath. U have Sjorgrens Syndrome. I take plaquinell and ibuprofen.

Where exactly is pain located?

Customer:

Just under my left shoulder blade.

Dr Jones :

You mean on side of your back?

Customer:

More towards the middle but not by my spine.

Dr Jones :

Customer:

If I double my arm back, I can touch it with my thumb.

Customer:

There is sharp pain if I try and take a deep breath. My elbow is sore as well.

Dr Jones :

Like short breath, fever, difficulty in urination or anything you feel that is abnormal?

Dr Jones :

Do you had any unusual physical activity recently?

Customer:

No - it has just gotten progressivley worse and now it is constant even when I try and take shallow breaths. It seemed to be much worse after I ate lunch.

Customer:

I have Sjorgrens Symdrome and take plaquinel and ibuprofen 2 x per day. I use a Duleral inhaler twice daily.

Customer:

I did feel like it was worse afte my pilates class on Thursday. I just kept thinking I had strained it some how. Then it became more than a dull ache and started being sharp pains.

Dr Jones :

Commonly pain in such location is caused by sore inter-coastal muscle.

Dr Jones :

But since it has become sharp now I guess this needs clinical evalaution.

Dr Jones :

Underlying lung and pleural causes can also mimic such pain.

Customer:

Something I can call my internist about on Monday am?

Dr Jones :

Sometimes referred pain from other organs can be felt in this area.

Customer:

OK - anything I should look for that would indicate I should do something before Monday am?

Dr Jones :

I guess if its only pain then you can wait till Monday.But if there is further progress in pain or other symptoms particularly related to lungs develop do consider a ER visit.

Dr Jones :

But since you have pulled on for three days I don't expect something extremely serious.

Thank you for your time and I will call my MD in the morning and if the pain get much worse - I think I will have to go to the ER. Thank you again!!
