You symptoms are not typical of one condition but cause which can present like this are kidney stone and sometimes inflammation of gall bladder can cause pain in this region.
Kidney stone can cause pain in this region radiating to lower back and thigh at times.
All cases of such abdominal pain need clinical evaluation and ultrasound of the abdomen.
I had my gall bladder removed last summer after I saw a doctor about this same abdomenal pain but the pain has gotten worse in the last month
Ultrasound can help to determine the cause of pain and help to plan treatment.
So clinical evaluation and ultrasound are desired investigation in your case.

The pain is worse when sitting and better when laying down
Then I would consider possibility of renal stone is foremost cause.
You don't think that my RA meds have damaged my liver?
Pain radiating to back is common symptom seen in obstructed kidney stones
But liver damage wont cause pain radiating lower back.
Pain would be located to right upper quantrant.

And ultrasound will also help to see if there are any liver changes.
So you will get both kidneys and liver evalauted.
The fatigue is what is bothering me the most... it is constant and extreme
Fatigue can be part of RA and its medicines.
