Pain between the shoulder blades can be caused by muscle strain of the back commonly.
This is plusible since it happended to you following the swimming.
Another possibility is underlying thoracic aortic disease.
Thoracic aorta is a big blood vessel.
An aneurysm or dilatation of this aorta can cause pain between the shoulder blades.
If you have history of hypertension or heart disease, this possibility should be ruled out with a Ct scan of the chest with contrast.
Otherwise, this is likely a muscle sprain.
you may consider chiropractor therapy along with heat application for recovery.
I do have a history of hypertension and take medication for it. I also have diabetes type 2 controlled with diet and exercise.
I will suggest to check out the aortic aneurysm possibility.
If that is negative this is then muscle sprain.

yes, this can also be seen with heart disease as well.
Since you are diabetic you need to be little careful with these symptoms.
The heart can be checked out with a EKG and cardiac enzymes.,

Yes i think you should see a doctor as soon as possible.
This is especially true since tylenol is not proving helpful.
