Run the downloaded program and after it is installed, open the program.
Connect your iPhone to your computer and from there, you can activate within iTunes.
iTunes does not have a password.
No, you should be able to activate it within iTunes.

How did you previously back up your iPad?
Yes, you would unless you restored from a backup.
If you backed up your iPad on a computer then you need to use that computer.
If you backed up the iPad using iCloud then you can restore with that without using a computer.
