What could be penalty who wrote restraint order

Customer Question

what could be penalty for doctor who wrote restraint order after the facts.
Patient was in restraint and nobody called doctor until patient remained in restraint for one hour. The staff intimidated doctor to write order and he wrote it. In routine doctor signs order within 5 to 10 minutes of start of restraint and he wrote the time according to routine when he was writing this order one hour after the start of restraint.
Nobody got hurt in this incident.
Doctor did not change or falsify record to hide any of his mistake.(it was staff mistake that they did not call doctor when they were supposed to call. )
Now doctor is being accused of falsifying record.

I think there is no harm here and no one was hurt the penalty would be minimal. He might be suspended by the licensing board but I suspect it would be brief.
