Yes, this is unprofessional.

Unfortunately the term "social worker" is very vague, and it covers both professionally licensed health care workers (who are regulated by health care privacy and confidentiality laws - most significantly "HIPPA" - which forbid this type of disclosure, and provide for significant penalties, see: http://www.hhs.gov/ocr/privacy/hipaa/complaints/), and it covers unlicensed, untrained, individuals who simply help in non-healthcare settings - these individuals may not be covered (although an affected client may still choose to file a HIPPA complaint - use the link above - in any event).
