affidavit with the Court. Normally, this is done at the beginning of the suit, but may be done during the suit as well, at the discretion of the Court.
The in forma pauperis affidavit is also called a pauper's oath, or, a pauper's affidavit. A generic sample (NOT Florida) may be seen here
One files an affidavit and asks the Court for an oral hearing to consider the matter. If the Judge agrees, fees associated with the suit may be waived.
