First: You must file the original Answer, Affirmative Defenses and completed Proof of Service with the court's clerk within 30 days after receiving the lawsuit paperwork. Forms that you will need:
Make 3 copies of your completed papers (one for Yourself. one for the clerk of court and one for the other party or their attorney). The original is filed with the court.
2. Serve the paperwork to the other side:
a. Have someone over 18 who is not involved in your case (the "server") mail one copy of the Answer, and Affirmative Defenses to the lawyer or the person suing you (if they do not have a lawyer) 1st class U.S. mail. The address of the lawyer or person suing you is in the Complaint. You cannot mail the papers yourself!
b. After the server mails the documents have him or her complete the Proof of Service by Mail form.
3. Bring the completed Proof of Service by Mail along with your original Answer and Affirmative Defenses to the court clerk and hand the documents answer, defenses, and proof of service to the clerk for filing. Make sure you bring 2 copies one for your records and one for the clerk.

If you have an Attachment 4, Title a Page Attachment 4, and attach right after PLD -C-010. Then put your signed and notarized verification form after the attachments. Then put the Proof of Mailing After the Verification.
