i just recieved a letter from IRS stating i had missed filing a 1099G from unemployment and that i either needed to pay a bill or i needed to amend my tax return
i am amending it on tuesday
You have two options, you can do nothing, or bring it to light. Since you filed your taxes and amended your return, then you satified the IRS, who is the main person you should be worried about.
i am more worried about the state finding out
they could prosecute me, i work for a bank now, i finally have my life straightened out
It is unlikely they will unless you are collecting unemployment still.

You could be forthcoming, but then they will likely make you pay it back.

The state can put you on a payment plan also. What is the amount they paid in unemployment?
i had unemployment again for 2012 (difficult last couple years) and i recieved one weeks wages for a job but took unemployment realized what i did and skipped the following weeks claim and of course once that w2 was filed the EDD found out and i paid it back and was banned from unemployment for 13 weeks
but in 2011 i did it for 27 weeks
will the state find out if my earnings were reported on a 1099 and not a W2?
Yes, you could contact the unemployment office and tell them that you misunderstood the criteria and need to remedy the situation. They will escalate it to their supervisor and you will have to fill out some paperwork.
The state will only find out if they audit you, which is rare.
Yes, it is rare. Usually only the tax people dig into that information.

It is, but they usually only charge the people they audit and find out about them, not those who come forward.

The odds are very low that you are going to be audited. I'd have to see your entire return to answer if you would be red flagged, but there are many reasons tax collectors audit. Usually in Utah it is because many people own their own businesses and incorrectly take deductions. You paid taxes on your income, so it wouldn't normally raise a red flag at all.
no the only deduction i have is around 100 to a charity i subscribe to
and i did put in an incorrect education deduction

I would suggest just waiting, unless it would make you feel better to square up with the state. If you do contact the state, there is little chance at getting jail time.If you do nothing, then odds are you will be fine.
