When filing a a certificate of incorporation in NY, you need a cover/back page that partly says "filed by" where you fill in a name and address. Should the attorney's name be there or the incorporator?
Optional Information: Country relating to Question: United States State (if USA): New York
Hello,See here for a sample filing:http://www.dos.ny.gov/forms/corporations/1239-f-l.pdfIt sounds like what you are talking about is the second page entitled Incorporator Information Required. Yes, that does need to be included.Other than that, nothing else beside a cover letter stating what you are filing, if you are filing this by mail. The incorporator's name should be there, not the attorney's.I hope this finds you well.
