The name would be simply Jane Doe. In the section where you are naming them, say "Defendant is Jane Doe, who resides at _________. Petitioner knows Jane Doe as "Lauren" but does not yet know Lauren's last name, and thus names her as a defendant to this cause as Jane Doe."
