Hi - I practice in Tennessee.

 

I would certainly recommend that you have an attorney in your area draft this for you as the document needs to be filed with the chancery court clerk as well.

 

However, you can find generic, non-specific POA's online. But, it's worth the money to get a specific document drafted that suits your needs.
