Is your divorce already finalized? Usually, you tell the court you want to go back to using your old name upon the issuance of the final divorce decree. If you did not do that, you will have to go to your local court house and ask the clerk of court for a petition to change your name. You fill that out and give it back to the clerk, along with any fee they might assess. Once the judge grants the petition, then you can take the Order down to the DMV, INS, Social Security, or whomever else and have your name changed on all your documents.
