Hi Judith, In line 14 of Form I-130: Date and place of admission

In line 14 of Form I-130: Date and place of admission for or adjustment for a lawful permanent residence and class of admission???

My green card shows Category E27, Residence since 05/04/05. but it doesn't show the place.

I485 Receipt Notice 10/29/2004 Derivative adjustment
I485 Approval Notice 05/04/2005 COA: E27

Below are my previous entries to US:

What should I wirte on this? I'm assuming 05/04/05, E27, but what is the place of admission?
