Not revving high in park is most likely just the rev limiter but obviously dying or not going over 40 MPH are definitely concerns. There are quite a few things that could cause these symptoms ranging from sensor issues, low fuel pressure, fuel contamination, cam timing, or an exhaust restriction.

Did the problem start not long after getting fuel?
What year and model is the vehicle and what engine do you have?
