I need to write an annotated outline for an APA style paper. I have looked online and even went to the library and could not find the information. It is for a paper for my degree in Healthcare Management.

Actually I have already gone to the library and I have the books suggested. They tell you how to write an outline which I am familiear with, but not how to write an annotated outline. I have asked teachers, librarians and searched the internet and no one seems to know.
