I have been married for over 30 years. I have had no sexual encounters outside of marriage during this time but about 32 years ago I did have some unprotected sex. My doctor is currently doing some some tests to discover cause of chronic diarrhea. He mentioned that HIV could be a cause. Naturally I am extremely worried now. My wife appears 100% healthy as are our three children. I had an HIV test about 20 years ago and the result was negative. Do you think there could be a possibility that I do have HIV?

No there is zero possibility

If you have tested negative 20 years ago and have been monogamous since there is zero chance

It is something else I'm sure that is causing this
