Well as you describe,possible causes may include
The worm movement sensations are called paraesthesia. These are due to nerve hypersensitivity which can happen due to neuralgia or nerve inflammation or even Vitamin B 12 deficiency,stress related,Urticaria, Morgellon's disease.
Further electromyography and nerve studies will give you a clue as to the cause.
A referral to either neurologist or Dermatologist can be considered.

As we discussed earlier various possible causes of such worm movement sensation on body which may require further investigations to determine the underlying cause.
Blood work is not alone sufficient to determine the underlying causative agent.
So I recommend you to see a dermatologist or Neurologist for further evaluation and management.
