I am sorry that your question was unanswered for long.
There are few possible causes for urinary smell in breath:
1) Chronic sinusitis
2) Nasal polyp
3) Gastroesophageal reflux disease (GERD)
4) Hiatal hernia
5) Dental and/or gum infections.
Any of these conditions can be concomitant.
Please feel free for your follow up questions.
I would be happy to assist you further, if you need any more information.
Thanks for using Just Answer.
All of these conditions aggravate during lying down (due to enlarged adenoids or tonsils etc) and can remain asymptomatic except with some unusual signs like what you had described. A physical exam and investigations would sure pin point the cause. This is unlikely to be anything serious. Some metabolic disorders rarely may also cause increase in substances which cause smell in breath. This can be detected by the blood work.
It is privilege assisting you.
