I'm 21 and my toes on my left foot have felt extremely cold internally, but when I touch them they feel normal. Is this something I should worry about?

In a young person it is unlikely to be worrisome. Still these symptoms warrant following causes to be looked for;

1) Pinched nerves in back; MRI of back is recommended for diagnosis.

2) Deficiency of vitamin B 12; blood work for its assessment.

3) Diabetes; if you have family history.

4) Peripheral arterial disease if you are a smoker, use tobacco in any form or take recreational drugs.
