it is likely you might.
important to get checked.
you may simply have a sinus infection at this point
or this could be developing into pneumonia
a chest xray and good exam will tell the tale
can you get in to see a physician?
with fever and chills my concern would be for an early pneumonia (lung infection)
though a sinus infection is certainly possible

do you have any chronic medical conditions such as diabetes or heart issues?

levaquin is not without it's risks but it sounds like you have taken it before
it is a fine antibiotic choice for upper respiratory infections
and if you have sinusitis or an early pneumonia it would be reasonable to try this
any way to contact your doctor back home?
and also, I'm not clear... have you taken levaquin before?
okay good, that takes the possibility of an acute allergic reaction to it off the table
so, your choice, I think it would be reasonable to take this
and to get a message to your doctors office telling them about your situation
when you can tomorrow
also, it would be reasonable to ask around where you are staying about acute medical care available in case you get worse while you are there.
