PHYSICAL EXAMINATION
General: In general, Mrs. Miller is a pleasant, white woman in no distress.
Eyes: Conjunctivae normal. Sclerae normal. Pupils equal, round, and reactive to accommodation. Fundoscopic exam normal. Oral mucosa hydrated. No ulcers. No aphthous.
ENT: Neck is supple. No JVD. No lymph nodes.
Thyroid: Nonpalpable.
Heart: Regular rate, normal rhythm. S1, S2 normal.
Lungs: Clear to auscultation.
Abdomen: Soft, nontender, nondistended. Bowel sounds are positive. No hepatosplenomegaly. There is Carnett's positive in the midline 3 cm below the xiphoid process.
Extremities: No clubbing, no edema, no cyanosis. Reflexes are intact.
IMPRESSION/REPORT/PLAN
#1 Chronic abdominal pain with recurrent gastric ulcers
The differential diagnosis for this chronic abdominal pain with recurrent gastric ulcers could be very broad. This could be peptic ulcer disease secondary to H. pylori and NSAIDs use, or it could be an upper GI bleed secondary to persistent purging and vomiting. For that reason, we are going to proceed with an upper endoscopy with biopsies for H. pylori for an infiltrative process of the stomach. We are also going to refer the patient to PMR for injection in the local site of the Carnett's positive point. We are going to refer the patient for a GI consult for her eating disorder. We have also recommended the patient to add fiber to her diet to try to have more regular bowel movements.

You're at the Mayo--one of the premier tertiary care and academic centers in the US. You've yet to be fully diagnosed and treated by your present GI. You need to let them do whatever they feel is necessary. End of story

Hard to say exactly--could be mastitis--infection--or hormonal but without being able to examine you I really can't say any more than of course, if it's still like that tomorrow, have your doc look at it

Pathology just came back from my scope


A. Stomach, Incisura Antrum, Body, endoscopic biopsy: Reactive
gastropathy. Antral and fundic mucosa shows reactive foveolar
hyperplasia, negligible inflammation, and no Helicobacter pylori.
These changes suggest chemical-type injury such as can be seen with
nonsteroidal anti-inflammatory drugs and bile reflux.
