How is the pus there in the first knot?
The new knot can be an enlarged lymph node.
This can arise from an infection or inflammation in its draining area.
As there was pus in the first one, it was likely to be an infection.
Did he take any treatment?
I would suggest to get an examination by doctor. He can need antibiotics for it.
He can take antiinflammatory analgesics like motrin or naproxen, if there is no other contraindication.
Those can help to reduce pain & inflammation.
He can also apply warm moist compress.

Please get him checked by doctor as it seems that the lymph nodes are also involved.
Start of antibiotic can help to control the infection.
