Based on your description, this is likely a bacterial infection.
An abscess formation is also possible.
A curse of antibiotic should be considered.
it is about 3"long and about 3/4 in wide and about 1/2 in high
and it is painful
along with fever.
That makes infection likely.
yes ,,hard to sit,,it is about 3/4 in from retrum hole running the lenghth of the inner cheek
this is concerning.
You should see your doctor soon to drain the abscess and also a course of antibiotic.
ER visit also will be appropriate for speedy treatment.

how big do they have to open the area
your size is adequate for drainage.
do they do it where it started
they open it enough to remove it completely.
that is at the top and it has grown downward
at the top.
it started up above ,,about 1" dia
yes it should be opened from above.
may be extended down to remove in entirety
do they have to stich it all up then
