At any point really but if the benefits outweigh the risk than that is what the decision will be based on.

I failed to mention that some studies showed that the risk of spontaneous abortion or miscarriage was higher in the First and Second trimester.

However the congenital anomalies did not increase significantly between moms who did have surgery and moms who didn't. Just that the risk of miscarriage was higher in the first and second trimesters.
