I have a head injury from a domestic situation and I am wanting to go to the doctor, because I am having blurred vision, headaches, nausea and my ears are ringing. I don't want to go if the doctor is going to report it to the authorities. Can they do that?

You don't have to tell the Dr how the head injury occurred but I would urge to do something about this...both your head injury and your situation.
