As such local infilterate in a lung may lead to formation of effussion.But in case of only a effussion there can be some other diseases like bacterial or viral infection,chemical inhalation induced,due to reasons related to the heart ,etc.
In the effussion there is edema in one side of the chest while it may or may not be in infilterate.Effussion may produce dull note on clinical examination while an infilterate is wide spread in nature & in case it is producing cavitaory lesions then it will produce resonant notes.
In the x ray the effussuin will be seen as generalized opaque shadow while an infilterate will be shown as cavitary lesion with illefined borders.
