i have very bad cyctic acne only around my cheek area, I feel it get's worse and more painful just before and during my menstrual cycle. I have a feeling that the acne is due to hormonal issues. I've tried many products including proactive, prescribed topicals and yasmin but it has not gone away. What do you recommend?

I've had acne for over 10 yrs it started as an adult right around 22 yrs old. I've been experience a bad outbreak (cyctic) about 8 months ago. I've used duac and tazorac. I've contsulted a dermatologists years back and doctors seem to overlook my problem.

There are some treatment options available for the acne.The common ones are antibiotics ointments, benzyl peroxide, oral antibiotics, isoretinoic acid application, laser, microdebrasion, cosmetic surgery, etc.
Since you have already used the topical cream like duac so you may consider the other therapy for the cystic acne containing isoretinoic acid like tazorac.The cystic acne is one of the most severe form of acne and isoretinoic acid application is very effective.It may take a few weeks to months before the cystic acne clears up.You may consult your dermatologist as it is available on prescription.The addition of oral antibiotics will help in the control of infection.
The other therapy which you may consider is to the laser treatment of these acne which is also very effective in treating the scar as well.The microdebrasion and cosmetic surgery are the other options.

so this wouldn't have anything to do with hormones?
The hormonal imbalanec may aggravate the condition and it is likley to be associated since your acne is more painful around the periods but since you have already tried yasmin which has not benefitted you.
The hormonal contraceptive like bc pills may be tried for the mild to moderate acne but usually not helpful in cases of severe cystic acne which you are having.
The iso-retinoic acid is usually prescribed for these severe cystic acne and also the pregnancy should be avoided when one is on the iso-retinoic acid therapy.
