no on the Ins Claim Form on line 21 you have to put the diagnosis or nature of illness the on line 24 E we have to put in the diagnosis pointer

That would be the codes...the ICD9 codes (which are the diagnosis codes)

Ok, I did a little research myself. The diagnosis pointer line has an arrow pointing downward to it, right? This is line 24 E....all you need to do is put either a 1, 2, 3 or 4 (this is section 21) in this box....depending on how it corresponds to what you put in boxes 1-6 (where you listed what was done....the part where you had to put in the service dates (24 A). Tell me if you follow me.

The diagnosis pointer will be either 1, 2, 3, or 4...whichever that diagnosis corresponds to on line 24 section 1-6. Say you put the CPT (procedure code) in for the shoulder xrays in on line 24D, you will have to have something shoulder related like "pain" as you mentioned previously somewhere in section 21....either 1, 2, 3, or 4 (and that will also need to be a code-the code for pain). Then say you put the diagnosis code (ICD9) code for pain on line 1, you would follow down and put a 1 in the diagnosis pointer section for that line that you listed the shoulder on.
